- Match with Your Consumer File – Please provide your full legal name (first, middle, last), any former names, and either your complete Social Security number or date of birth.
- Additional Identity Documentation – You may also submit government-issued ID (e.g., driver’s license, passport), recent utility bills, or other documents to prove your identity. You may also be asked to answer security questions related to your information.
Submit Proof of Human Trafficking Victim Status To support your request, you need to provide documentation proving your victim status, such as:
- A formal determination from a government entity (Federal, State, or Tribal authority).
- A statement from a non-governmental member of a human trafficking task force or an authorized victim service provider.
- A self-attestation signed or certified by a Federal, State, or Tribal entity, a court, or an authorized representative, confirming your status as a victim of human trafficking.
Identify Specific Information to Be Removed Clearly point out the specific information on your background report that is related to human trafficking and should be blocked. This helps us to address only the necessary items.

Our Information Blocking Process
Once we receive your request, Trustania will take action to block the requested information within four days. If we require additional details to process your request, we will contact you within five days. Our team will make a final decision within 25 days. We will inform you of our decision within five days, specifying whether the information will remain blocked or be restored.
